Parents of children who will be attending school at Fay Galloway Elementary School are invited to enroll their child in the school office. All registration requirements must be met Before your child will be placed on a class list.
Please bring your child’s original state birth certificate, immunization record, parent photo I.D. and one proof of address.
Nevada State law requires a child to be five years old on or before September 30, to attend Kindergarten. The following immunizations are also required by law:
DTaP 5 doses
POLIO 4 doses
MMR 2 doses
HEP A Series of 2 shots
HEP B Series of 3 shots
VARICELLA (Chicken Pox) 2 doses
