Enrollment:
Transferring from another school within the Clark County School District

1. Two Proofs of Address:
Proofs of address MUST be in parent or guardian's name.
- Power, water, or gas bill
- And/or rental agreement or
- Mortgage statement
NOTE: If you only have a sales contract for the purchase of a new home as proof of address, the school will enroll your child on the condition that one additional proof of address from the above list is submitted within 30 days. Driver's license, telephone bill or cable bill are NOT acceptable for proof of address.
***If you are currently residing with a friend or relative, a notarized statement signed by the owner of the residence is required. Include two proofs of address in their name. Additionally, your photo ID must bear the address of the friend or relative.

3. Withdrawal Form from previous school (If applicable - only required during the school year)
4. Completed and signed registration forms:

Immunization Records Required for Enrollment
|
Nevada law requires that children be current on their immunizations before they are allowed to enter school. Parents should bring immunization records with them to school when they register their child.
A child will not be allowed to attend school until the documentation is provided. Exceptions are permitted based on religious belief or medical condition, which requires a letter from a health care provider.
- Four (4) DPT Shots (Add one more if last dose was given before fourth birthday.)
- Three (3) Polio Doses (Add one more if last dose was given before fourth birthday.)
- Two (2) MMR Shots (one dose must be given on or after the first birthday. A second dose is required for entry into kindergarten and first grade.)
- Two Hepatitis A Shots (2nd dose must be given at least 6 months after the 1st dose.)
- Three Hepatitis B Shots (Must have a minimum of 4 months between 1st and 3rd dose.)
- Chickenpox (Varicella) shots (2nd shot only required is 1st shot given after the age of 13 years. Minimum of two months spacing between shots required.)

Title I Homeless Outreach Program for Education (HOPE)
|
If you are:
- Living in a multi-family situation due to loss of housing, economic hardship, or a similar reason.
- Living in motels, hotels, trailer parks, or camp grounds.
- Living in emergency or transitional shelters.
- Abandoned or who have run away.
- Awaiting foster care placement.
- Staying in a primary night time residence that is a public or private place not designed for, or ordinarily used as, a regular sleeping accommodation for human beings.
- Living in cars, parks, public spaces, abandoned buildings, substandard housing, bus or train stations, or similar settings.
You are entitled to immediate enrollment at your school of origin. Please advise the Registrar so the required paperwork can be completed.

Withdrawing Your Child From School
|
- Parents must fill out a withdrawal request paper found in the Registration Office.
- Your photo identification is required to withdraw a student from school.
- The student must remain in school (1) one entire day to go through the withdrawal requirements.
- Classroom books must be turned in to all teachers.
- Library books must be turned in to the library before withdrawal can be completed.
- The completed pupil release and transfer form will be given to the student to take home at the end of the school day. You will need to take the necessary documents to enroll the student in another CCSD school.
