By choosing the Eligibility Check option you affirm that the Human Resources Unit has notified you that an Eligibility check is required for contractor services. Refunds will not be issued under any circumstances. Proof of payment is required prior to being fingerprinted. 

A valid Government issued photo identification is required to be fingerprinted.  Acceptable documents include driver's license, state ID card, passport/passport card, military ID.
